Notice: Submitting this form does not establish a client relationship or initiate investigative services. Upon submission, this form becomes part of the official Retainer Agreement. Services will only begin after BOB’s PI & Company reviews all requests for legal and ethical compliance and formally accepts the case. A signed agreement is required before any investigative work will commence.
